RN Director of Case Management
Tampa, Florida
The RN Director of Case Management leads with executive presence and clinical credibility, builds trust across the leadership team, and develops a department that delivers results. The Director is a self-starter who manages competing priorities with minimal supervision and brings the change management skills that make improvement last.
The Director navigates conflict and offers the chance to own every aspect of a hospital Case Management department with the backing of the organization.
Qualifications:
- Registered Nurse (RN) required
- Bachelor Degree required
- Master Degree in Nursing, Health Administration or Business Administration preferred
- Director of Case Management experience in an Acute Hospital Setting required
- Certification in Case Management (ACM/CCM) preferred
Responsibilities:
- Provides overall leadership for all daily hospital Case Management operations, including Care Coordination, discharge planning and Utilization Management
- Directs and evaluates the Case Management model, staffing, skill mix, onboarding, technology and staff competencies to meet performance and quality goals
- Allocates resources to staff the department effectively and achieve productivity targets
- Ensures Care Coordination and discharge planning processes are timely, high quality and properly documented
- Partners with leadership to achieve established goals
- Improves department performance by analyzing data, evaluating processes and leading sustainable performance improvement
- Ensures compliance with organizational policies, standard operating procedures and external regulatory requirements
- Builds strong working relationships with the C-Level, ancillary service leaders, Ethics and Compliance, and Legal
- Coordinates with hospital departments and external agencies to support continuity of care and optimal resource utilization
