Position Overview
We are seeking a Licensed Practical Nurse (LPN/LVN) to provide nursing care to residents in a long-term care and rehabilitation setting. The role involves supporting residents with daily care needs, including feeding assistance and tube feeding, while coordinating with nursing staff and certified nursing assistants.
Required Qualifications
Active LPN/LVN license preferred; RN candidates may be considered if necessary.
Experience caring for residents in long-term care and rehabilitation settings.
Experience with American Data ECS charting is a significant plus.
Ability to provide resident care in a setting that includes feeding assistance, tube feeding, and mechanical sling lifts.
Responsibilities
Provide nursing care to residents in long-term care and rehabilitation settings.
Support residents who require hand feeding or tube feeding.
Assist with resident transfers and mobility using mechanical sling lifts as appropriate.
Monitor resident conditions and report changes to the appropriate healthcare professionals.
Maintain accurate clinical documentation using the facility's charting system.
Coordinate with CNAs and other nursing staff to support resident care.
Follow facility policies, infection prevention practices, and resident safety standards.
