This role is ideal for someone who understands Chicago’s communities, knows how to build genuine professional relationships, and can turn community engagement into meaningful connections and qualified referrals.

What You’ll Do

Build and maintain relationships with schools, hospitals, healthcare providers, housing programs, social-service agencies, faith-based organizations, nonprofits, and other community partners.

Educate community members and referral partners about Empower’s mental health services and eligibility requirements.

Identify prospective clients and connect interested individuals with the appropriate intake or enrollment contact.

Develop new referral sources and maintain consistent follow-up with established community partners.

Research local events, organizations, and partnership opportunities throughout Chicago and Cook County.

Track outreach activity, community contacts, referrals, and partnership outcomes.

Collaborate with leadership to strengthen Empower’s visibility and outreach strategy in the Chicago market.

Represent Empower professionally while protecting client confidentiality and following applicable ethical and compliance requirements.

What You’ll Bring

Strong knowledge of Chicago and Cook County communities, organizations, and available resources.

Demonstrated ability to develop professional relationships and generate referrals or community engagement.

Strong communication, networking, organization, and follow-up skills.

Ability to work independently, take initiative, and remain accountable for outreach results.

Reliable transportation and the ability to travel throughout the assigned service area.

Availability to attend community events, including occasional evenings or weekends.

Nice to Have

Existing relationships with Chicago-area schools, hospitals, shelters, healthcare providers, community organizations, or social-service agencies.

Experience promoting behavioral health or Medicaid-funded services.

Experience helping individuals navigate intake, enrollment, or service-referral processes.

Help Empower establish a meaningful presence in Chicago while connecting individuals and families with mental health services that can make a lasting difference.
