About the Role

We are seeking an experienced Registered Nurse Clinical Manager to lead and oversee clinical operations in a home health setting. The Clinical Manager will be responsible for coordinating patient care services, ensuring regulatory compliance, supervising clinical staff, and maintaining the highest standards of patient care.

Key Responsibilities

• Coordinate and oversee all direct patient care services provided by clinical personnel.
• Conduct and delegate patient assessments and reassessments.
• Develop, review, and update patient care plans in accordance with physician orders and regulatory requirements.
• Determine patient eligibility for admission and assign appropriate clinical staff.
• Collaborate with physicians, patients, caregivers, and interdisciplinary teams to ensure quality care and positive patient outcomes.
• Supervise and support clinical staff, including performance management, training, and development.
• Participate in interviewing, hiring, onboarding, and mentoring clinical team members.
• Review and audit clinical documentation, including Start of Care (SOC), Resumption of Care (ROC), and recertification assessments.
• Ensure compliance with Medicare Home Health regulations and quality standards.
• Monitor OASIS documentation, submission processes, and regulatory requirements.
  

Requirements

- Graduate of an accredited School of Nursing
- Active Registered Nurse (RN) license in Florida or compact state eligibility
- Current CPR certification
- Valid driver's license, auto insurance, and reliable transportation
- Minimum 2 years of RN experience
- Minimum 1 year of management experience in Home Health, Hospice, or a similar healthcare setting
- Strong knowledge of Medicare Home Health regulations
- Hands-on experience with OASIS assessments, documentation, and compliance (Required)
- Excellent leadership, communication, and organizational skills
