Part-Time Field Case Manager
Our growing guardianship company is looking for a compassionate, dependable Field Case Manager to join our team on a part-time basis. In this role, you’ll be the eyes and ears for the people we serve: visiting clients in person, evaluating their well-being, and keeping their assigned Case Manager informed so every client gets the care and support they need.
What You’ll Do
- Conduct thorough, meaningful in-person visits with each assigned client
- Assess clients’ living conditions, including safety and cleanliness of their environment
- Evaluate clients’ physical and mental health
- Identify any unmet needs and recommend appropriate follow-up
- Report findings clearly and promptly to the client’s assigned Case Manager

- Strong organizational skills, with the ability to prioritize and manage multiple visits
- Detail-oriented and committed to meeting deadlines
- Excellent written and verbal communication skills
- Dependable, honest, and trustworthy
- Must be able to pass a background check
- Experience working with vulnerable adults is preferred
