Benefits:About the Role: Karing Hearts Home Health is looking for a compassionate and skilled Per Diem RN to join our home health team in Carlsbad, CA. This is a fantastic opportunity for a registered nurse who values flexibility while making a meaningful difference in patients' lives right in the comfort of their own homes.
Responsibilities:- Conduct skilled nursing assessments and develop individualized patient care plans
- Administer medications, wound care, IV therapy, and other clinical treatments in the home setting
- Monitor and evaluate patient progress, adjusting care plans as needed
- Educate patients and family members on disease management, medications, and home safety
- Coordinate care with physicians, therapists, and other members of the interdisciplinary team
- Accurately document patient visits, clinical findings, and care delivered in compliance with regulatory standards
- Respond to patient needs and changes in condition with timely clinical intervention
Requirements:- Active California Registered Nurse (RN) license in good standing
- Minimum 1 year of clinical nursing experience; home health experience preferred
- Current CPR/BLS certification
- Valid California driver's license and reliable personal transportation
- Familiarity with OASIS documentation and Medicare home health guidelines is a plus
- Strong clinical assessment, critical thinking, and communication skills
- Compassionate, self-motivated, and comfortable working independently in the field
