Social Services & Admissions Coordinator Role:

• Responsible for assisting in growing the facility's census.

• Manage the admission process by maintaining Room Control, timely follow up, and manage occupancy levels. Complete admissions paperwork.

• Oversee the development, implementation, supervision, and ongoing evaluation of the Social Services Department.

• Meet with and assist residents and their families with medically related services, grievances, or other placement needs to help maintain the highest quality of care.

• Complete assessments and participate in comprehensive care plans.

• Facilitate residents’ safe transition upon discharge as well as facilitate transportation for appointments or other scheduled events.

• Maintain confidentiality of protected health information, including verbal, written and electronic communications.

Social Services & Admissions Coordinator Requirements:

• A minimum of a bachelor’s degree or related experience in social work or healthcare admissions

• Nurse experience as a LPN also preferred (current unrestricted LPN license in the state of Tennessee)
