Registered Nurse RN

Signs of Life 2, LLC is seeking a compassionate Registered Nurse (RN) to provide community-based nursing services to adults enrolled in the Home and Community-Based Services–Adult Mental Health (HCBS-AMH) program in Houston and Harris County.

Use your nursing skills to help participants understand their health needs, manage their wellness, and work toward greater independence.

Position Details

• Pay: $43 per hour

• Subcontractor opportunity

• In-person services in Houston and Harris County

Responsibilities

• Complete nursing assessments and monitor participants’ health needs.

• Provide education about medications, health conditions, and wellness.

• Identify and communicate changes in participants’ health.

• Coordinate with service providers and the participant’s recovery team.

• Complete accurate, timely nursing documentation.

• Provide services according to each participant’s recovery plan and applicable nursing standards.

Qualifications

• Current RN license authorizing practice in Texas.

• Current CPR/BLS certification.

• Strong communication, organization, and documentation skills.

• Reliable transportation to participant visits.

• Mental health or community nursing experience preferred.

• Ability to complete required background screening, training, and onboarding requirements.
