ABOUT THIS APEX PROGRAM COMMUNITY SUPPORT INDIVIDUAL ROLE

This is a full-time, onsite position based in the Crisp County School System in Cordele, GA, working a dayshift schedule.

As an APEX Program Community Support Individual, you will provide case management services and crisis intervention for youth with behavioral health disabilities within the Crisp County School System, as well as in their homes and communities. You will help individuals function more safely and independently while serving as a positive role model and source of support. Throughout the day, you will identify social, economic, recreational, and educational resources that meet client needs and maintain liaison relationships with schools, community organizations, professionals, government officials, and other agencies.

You will monitor treatment and medication compliance, assist with behavior management training and skill building for both individuals and families, and provide emotional support and feedback. You will recognize and respond appropriately to potential crises, communicate concerns to therapists and supervisors, and contribute to the development and evaluation of treatment plans. You will monitor client progress, recommend modifications to plans of care when appropriate, maintain timely documentation, participate in interdisciplinary team meetings and supervision sessions, meet billing requirements, and manage a caseload of up to 50 clients. You will also assist individuals in attending appointments and provide transportation when necessary while ensuring compliance with all transportation policies and requirements.

WHAT WE'RE LOOKING FOR IN AN APEX PROGRAM COMMUNITY SUPPORT INDIVIDUAL

• High school diploma or equivalent
• Valid, unrestricted Georgia driver's license
• Computer literacy
• Ability to transport individuals served when required
• Ability to recognize potential crisis or emergency situations and respond appropriately
• Ability to maintain accurate and timely documentation
• Ability to collaborate with therapists, treatment teams, families, schools, and community partners
• Ability to manage a caseload of up to 50 clients
• Commitment to supporting youth with behavioral health disabilities in school, home, and community settings
• Bachelor's degree in a related field (preferred)
• 2+ years of experience working with youth and/or individuals with behavioral health disabilities (preferred)
