Agawam Campus is seeking a PRN Physical Therapist (PT) to join our rehabilitation team within a skilled nursing facility. This role offers flexible scheduling and the opportunity to work with residents focused on improving mobility, balance, strength, and overall functional independence.
Position Responsibilities:
- Complete physical therapy evaluations and provide individualized treatment sessions
- Address gait training, transfers, balance, strengthening, endurance, and mobility goals
- Collaborate with rehab staff, nursing, and physicians to support resident care plans
- Monitor resident progress and adjust treatment plans as needed
- Maintain timely and accurate documentation

Qualifications:
