Role and Responsibilities

A qualified professional, who directs, supervises,evaluatesandprovidesphysical therapy services to patients in the home,following physician’s orders.

Essential Functions

• Evaluate patient using diagnostic muscle, nerve, joint,and functional abilitytestsand measures.
• Prepare an assessment of the patient’s condition at the time of theinitialevaluation and perform periodic re-evaluations, develop a written problem list and plan of care including types and frequencies of treatment.
• Recommend modification of plan of careto physician,as needed.
• Document clinical notes on the date of the visit into medical record and keep all records and summariesup-to-date.
• Treat patients to address impairments and functional limitations, with a goal ofreachingmaximum functional independence.
• Instruct and/or recommend patients and their caregivers in the use of prosthetic, orthotic,and durable medical equipment (canes, walkers, wheelchairs, etc.).
• Include and educate thepatient andcaregiver(s)regardingtreatmentplanand goals.
