You belong here!We are looking for a Certified Diabetes Educator at our East Baltimore Medical Center practice in Baltimore, Maryland. In partnership with Baltimore Medical System another local health system, that provides much-needed healthcare services to the community. Under general supervision, assesses, plans, implements, and evaluates several components of the Johns Hopkins Medicine diabetes education program. Also serves as the diabetes nutritionists for patient education. Plans and conducts continuing education programs in diabetes and nutrition for allied professionals. The Diabetes Educator will provide diabetes education to patients and their families in an outpatient setting. The educator completes comprehensive diabetes assessments for each patient including emotional and behavioral health, interprets personal health data, develops an individualized care plan based on the patient’s assessed needs and goals and promotes successful self-management. Eager St., Baltimore, MD 21202Shift: Exempt - Full-time - 40 hours per week What You'll Need to be Successful:A Bachelor’s degree in Dietetics or Masters degree in Social Work or related field required. Current license or registration in specific allied health profession (e.g. Registered Dietician, Registered Nurse, Licensed Clinical Social Worker) required. Must obtain Certified Diabetes Care and Education Specialist (CDCES) within 1 year of employment. Knowledge of medical terminology, diagnosis, and basic disease processes. Analytical ability required to gather data, decide on conformity based on predetermined criteria, identify problems and refer for resolution. Basic knowledge of medical records coding standards. Awareness of licensing and accreditation standards. Knowledge of MS PowerPoint, Excel, and Outlook. Knowledge of relational EMR data documentation, ability to conduct research of issue at hand, as well as formulate recommendations based on findingsRequires a minimum of 2 years’ experience as CDE. Salary Range: Minimum $33.61 per hour - Maximum $55.51 per hour.
