SUMMARY
As a Physical Therapist with Home Care, you will provide comprehensive and compassionate skilled therapy assessments to assigned clients.
STATUS:Full Time- Weekend enhanced Rate
LOCATION: Monroe County
DEPARTMENT: Rochester Regional Health Home Care
SCHEDULE: 8AM-6PM (Saturday & Sunday); third day is negotiable
ATTRIBUTES
Current New York State license or a limited permit as a Registered Physical Therapist.
Current and validNYS Drivers license.
Master’s Degree in Physical Therapy or Bachelor’s degree with prior experience as a licensed Physical Therapist required.

RESPONSIBILITIES
Patient Evaluation. Performs skilled therapy assessments and prescribed treatments to assigned clients based on MD orders.
Treatment & Planning. Orders, obtains prescribed equipment as needed, and teaches clients regarding the equipment obtained. Communicates plan of care to the Primary Care Physician (PCP), and updates the PCP with changes in condition and subsequent alterations to the plan of care.

BS: Physical Therapy, DPT
LICENSES / CERTIFICATIONS:
PT - Physical Therapist - New York State Education Department (NYSED), RN - Registered Nurse - New York State Education Department (NYSED)
PHYSICAL REQUIREMENTS:
M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

$70,700.00 - $94,300.00

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
