Job Summary
- The Social Worker is accountable for the planning, organization and implementation of social work services to patients and families in the clinic setting, assuring that medically related emotional and social needs are addressed on an individual basis for the patient.
- The Social Worker is responsible to assess needs of patients facing chronic illness and facilitate plans with patients and families to promote effective care.
- The social worker will maintain a current knowledge base of available community agencies and resources to facilitate appropriate referrals.
- Provides direct clinical therapeutic services to patients as part of an overall comprehensivepatient care plan.
Education and/or Experience
- Master’s Degree in social work required from a program accredited by the Council on Social Work Education.
- 1-2 years’ healthcare social work experience is required.
- 3-5 years’ experience is preferred.
Licensure/Certification/Registration
- Certified Social Worker (CSW-PIP) License in the State of South Dakota required. (Multi-state licensure may be required)
- Quality Mental Health Professional Credential (South Dakota) preferred.
Work Schedule
- Monday – Friday, 8:00am - 5:00pm; 80 hours every 2 weeks

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
