Position Summary:
We have an exciting opportunity to join our team as a Clinical Coordinator Cardiology.

In this role, the successful candidate As the Heart Failure Care Coordinator, you will play a pivotal role in overseeing the comprehensive care of heart failure patients from first consult through post-discharge. Your responsibilities will include assessing and planning individualized care during the inpatient phase, coordinating discharge plans, providing education to patients and families, and acting as a patient advocate throughout the continuum of care. Additionally, you will collaborate with interdisciplinary team members, manage administrative tasks, and ensure quality outcomes through continuous monitoring and improvement efforts.

Job Responsibilities:

    1. Coordinate clinical care for heart failure patients across the care continuum, from admission to post-discharge follow-up.
    2. Communicate effectively with patients, families, and caregivers to ensure understanding and compliance with care plans.
    3. Deliver comprehensive education to patients, families, and nursing staff regarding heart failure management and self-care strategies.
    4. Provide daily administrative oversight of the heart failure care program, including managing resources, scheduling, and documentation.
    5. Lead and monitor care protocols, quality outcomes, and identify opportunities for process improvement.
    6. Coordinate discharge medications, including assisting with prior authorizations and ensuring continuity of care.
    7. Conduct post-discharge follow-up calls to assess patient progress, address concerns, and facilitate necessary follow-up appointments.
    8. Provide genetic counseling assistance related to heart failure.

Minimum Qualifications:
To qualify you must have a Bachelors Degree required Masters degree preferred Four to five years of recent full time Nursing experience required with teaching experience preferred. The Specialty Instructors must have two to three years recent experience in the respective specialty. (i.e.: Critical Care, Maternal Child, Perioperative).Demonstrated patient care competency.Previous involvement in orientation process preceptorship/internship programs.Demonstrates an understanding of the teaching-learning process.Demonstrates commitment to continuing professional development.Licensed and currently registered as a Professional Nurse in NYS.

Preferred Qualifications:
Bachelors Degree required
Masters degree preferred
Four to five years of recent full time Nursing experience required with teaching experience preferred.
Two to three years recent experience in the respective specialty: Cardiology.
Licensed and currently registered as a Professional Nurse in NYS.
Experience in program development, data analysis, and a strategic mindset akin to a marketing manager
Demonstrated ability to oversee in-house operations, manage patient experiences, monitor metrics, and communicate with outpatient and referring physicians
Commitment to delivering high-quality patient care and continuous improvement initiatives

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $95,637.61 - $137,800.00 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
