POSITION SUMMARY: The Assistant Community Care Manager will provide support to Community Manager (s) with planning, directing and organizing the delivery of LIfeStyle Options services in a manner that ensures quality standards. This is a great opportunity for candidates that have a CNA, looking to get into management and have previous home care and care coordination experience.

ESSENTIAL DUTIES:

• Care coordination for residents in retirement community
• Monitor Care plans to ensure resident needs continue to be met.
• Coordinates client weekly billing and transmittal
• Conducts client evaluations
• Fill-in clients services as needed.
• Conducts caregiver orientation/training, as needed
• Coordination of client and caregiver schedules, approval of timeslips and entering corrections/changes in the scheduling software.
• Caregiver coaching and counseling as required
• Performs the duties of the Community Manager in their absence.
• Other duties may be assigned

POSITION REQUIREMENTS & COMPETENCIES:

• Certified Nurse’s Aide in good standing with the state is preferred
• Two (2) years health care experience preferred.
• Ability to work independently
• Must have a valid driver’s license and reliable vehicle
