This APRN will be responsible for:
  • Joining an interprofessional specialized team consisting of Orthopedics, Neurology, Neurosurgery, Complex Care, Physiatry in the provision of family-centered care in treating children across the spectrum of Cerebral Palsy. Participating in interdisciplinary team processes and conferences identifying individualized approaches and planning for patients within the CP Center.
  • Enhancing continuity of care to include triage of new and second opinion patients, facilitating and coordinating a comprehensive plan of care in collaboration with patients and families to provide anticipatory guidance for their medical and surgical care needs across the continuum.
  • Functioning in a direct care role within the Multidisciplinary Clinic, assessing and treating new, returning, and urgent care access for patients in collaboration with the Complex Care Service or other Cerebral Palsy Center Physicians. Supporting population management to high-risk patients including ensuring recommended surveillance and follow up. Supporting perioperative process with families including meeting with surgical candidate patients and educating families and managing expectations around perioperative experience.
  • Applying clinical knowledge and assessment for ongoing management of patients, to include patient education, applying and developing evidence-based practice and research to achieve the best possible patient experience and outcomes.

To qualify, you must have:
  • A Master’s degree in Nursing
  • A current MA NP license (FNP or PNP)
  • Strongly Preferred experiences include: 5+ years of pediatric RN experience 2-3 years of Advanced Practice experience assisting in the care coordination of patients with complex medical conditions
  • Ability to work (5) 8 hour days, M-F
