Summary

Position Summary:

  • Performs routine patient care procedures as determined by the plan of care and directed by the Registered Nurse.

Essential Functions:

  • Safely transfers and transports residents.
  • Prepares resident for transport.
  • Maintains safety and confidentiality.
  • Resident Customer Service
  • Acts as a Resident Champion to advocate and escalate concerns.
  • Prepares the participants for meals. Serves and removes food trays and assists with meals.
  • Respond to Call Bells
  • Provides personal care services which may include oral hygiene; manicures; shaving and hair care
  • Reports observations of participants’ performance behaviors and actions and any changes to the
  • nurse
  • Communicates effectively and establishes good relationships with personnel of unit, resident/patient,
  • family and other departments, visitors and physicians.
  • Recognizes hazards to resident/patient safety and takes appropriate action to maintain a clean and
  • Safe environment.
  • Assists in preparation for new admissions prepares room, bed, equipment in advance.
  • Assists in transfer of patients to another room or another unit.
  • Assists and accompanies patients in the discharge procedure.
  • Provides postmortem care per policy.
  • Basic Nursing Care
  • In a team relationship with the preceptor and/or nurse, provides basic nursing care to patients.
  • Provides AM/PM care, which may include bed bath, oral hygiene, combing hair, shaving, dressing residents, changing bed linen, cleaning overbed table and bedside stand and straightening room.
  • Provides turning and positioning every two hours when indicated.
  • Performs routine treatment procedures including application of devices for heat, cold, etc.
  • Takes temperature, pulse, respiration, weight, height and blood pressure accurately and in a timely manner. Performs re-takes without reminder.
  • Prepares patient for meals. Assists with meals or feeds patients.
  • Distributes drinking water and nourishments to patients in a timely manner.
  • Answers call lights in a timely manner. Anticipates patient needs. Makes rounds of assigned patients.
  • Assists with handling of care of clothing and other personal property (glasses, dentures, contact lenses, hearing aids and prosthetic devices, etc.). Collects and transports specimens according to

procedure.

  • Maintains a safe environment for the resident: bed in low position, siderails up per policy, call light within patient's reach, etc.
  • Performs ROM, ambulation, wheelchair mobility, bowel and bladder training.
  • Serves and removes food trays in a timely manner.
  • Obtains temperature, pulse, respiration, blood pressure, height and weight for new admission upon arrival.

Communication

  • Communicates essential information in an accurate and timely manner.
  • Readily assists co-workers and maintains a cooperative and helpful working relationship.
  • Follows direction from Manager, RNs, LPNs and preceptor in a positive manner.
  • Communicates problems, concerns and issues interdepartmentally and interdepartmentally in an appropriate manner.
  • Responds to patient requests promptly and courteously.
  • Responds to requests from peers and supervisors promptly and courteously.
  • Promotes teamwork.
  • Provides assistance with patient admission, discharge or transfers.

Level of Supervision

  • Required: Works under constant supervision
  • Contacts: Regular contacts, within or outside the Company to give or get information.
  • Require courtesy, tact, and some knowledge of Company procedures.

Qualifications/Requirements

Entry level knowledge; basic employment skills. Degree or HS Diploma not

Education/Knowledge:

  • Experience: Less than one year
  • Technical/Clinical Skills (Usage):

Certifications, Licenses, Registrations:

  • American Heart Association CPR Certification
  • Must pass the GNA exam within the first 90 days
  • Must be a graduate of an accredited CNA program
  • CNA license or 90 day Temporary CNA license must be posted to the Maryland Board of Nursing Site
