Clinical Dietician – Cardiac Rehab
Day Shift - 24hrs/week - Flexibility with Schedule - No Nights, Weekends or Major Holidays!
Who We Are:
Cardiac rehabilitation is a medically supervised program to improve your health after a heart attack or heart surgery. It features individually tailored exercise, education, and counseling. We are certified by the American Association of Cardiovascular and Pulmonary Rehabilitation, and our cardiac rehab team includes doctors, nurses, physical and occupational therapists, and dietitians.
About the Job:
Provides nutritional care to patients in cardiac and pulmonary rehabilitation through medical record review, clinical assessment, diet education, communication with nursing, physician and ancillary staff.
What You’ll Do:
- Reviews patient medical record to formulate a nutritional assessment. Consults with health care team to determine the best method of nutritional care/support.
- Performs comprehensive dietary intake assessments and provides one to one nutrition counseling.
- Develops a written report with recommendations based on initial and follow up counseling sessions.
- Provides education and counseling on specific dietary modifications needed to achieve target goals.
- Collaborates with Medical Director to identify the best method of nutritional care/support.
Who You Are:
- Bachelor’s degree in nutrition and dietetics from an accredited program; Academy of Nutrition and Dietetics (AND) accredited dietetic internship.
- Maintains status as Registered Dietitian and is licensed in the state of New Hampshire.
- Knowledge of role and impact of diet on CVD progression and risk factor management. Knowledge of impact of diet on chronic diseases. Demonstrates knowledge of principles of weight management through the balance of caloric intake and caloric expenditure.
- Skilled in basic computer using Word, Excel, and Power Point
- Ability to:
- Demonstrate knowledge of current guidelines and recommendations for healthy body weight.
- Demonstrate competency in essential job skills.
- Be able to prioritize in caring for multiple patients.
- Ability to communicate verbally with customers in person or on the phone.
