For more than 118 years, Backus Hospital has been the primary source of healthcare services for much of Eastern Connecticut. Job Summary

The regional Social Worker will provide complex social work services as a member of the care management and interdisciplinary professional team at Backus Hospital and Windham Hospital. The regional Social Worker will demonstrate strong advocacy skills; focusing on problem solving and collaborative plans for positive patient outcomes and transitions. This role would focus on complex patients both social and medical with an anticipated long length of stay and complex transition planning needs.

Qualifications

Education: Masters degree in Social Work (MSW) required.

Experience:Minimum of five (5) years’ experience in hospital based social work or care transitions.

Licensure, Certification, Registration: Licensed Clinical Social Worker (LCSW); would consider licensure in progress

Language Skills:Excellent written & verbal skills.
