Job Overview:
The Licensed Practical Nurse is a technical caregiver who administers nursing care under the direct supervision of a registered professional nurse. Provides direct patient care to patients under direct supervision of a registered professional nurse and / or physician. Works with patients/family and other team members in providing age appropriate health care needs. Practices within the guidelines of the State Nurse Practice Act. Performs all other duties as assigned.
Qualifications:
Required:
- Graduated from an accredited program of Practical Nursing
- Current New Jersey LPN license
- BLS Certification through American Heart Association only
- Successful completion of all Orientation Programs
Preferred:
- Minimum 2 years experience in related health care field or Public Health
Certifications and Licenses:
- BLS certification through American Heart Association.
- Current New Jersey LPN license
Scheduling Requirements:
- Days
- Monday & Tuesday 10a-6p
- Wednesday & Thursday 9:30a-5:30p
- Friday 9a-5p
- Full-Time (37.5 hrs)
Essential Functions:
- Able to manage and provide safe nursing care in a high volume practice and demonstrates a positive, professional manner
- Completes assigned tasks (i.e. supply orders, medication logs, code cart check, refrigerator temp log and monthly reports) in the time allotted and submits to appropriate staff
- Coordinates ordering supplies pertaining to clinical area to promote smooth and efficient operations
- Demonstrates excellent communication skills ie phone, with patients, visitors, staff and providers etc.
- Documents the appropriate intervention plan and evaluation to reflect visits in a clear and concise manner under the supervision of an RN and or MD
- Document updates the medical record as required including but not limited to, intake forms within 3 visits, allergies, summary list, medication list , immunization list, education form etc.
- Gathers information, performs technical aspects of patient care (i.e., vital signs, injection, dressing change) according to established professional guidelines and communicates to RN and/or MD or team as appropriate
- Administers medication safely according to policy and documents same. Chaperones patients as indicated. As part of the team, coordinates patient follow up of appointments, tests and plan as per the provider s instructions. Documents care and follow up in the medical record
- Maintains the NJ standards of nursing practice and implements the policies and procedures
- Integrates the Nursing process utilizing a multidisciplinary approach in the delivery of patient care and identifies self-care deficits
- Participates in Quality Improvement processes and identifies and implements potential corrective actions. Submits monthly audits timely and accurately
