RN Care Coordinator (Longitudinal)
Day Shift (Hours: 8:30a– 4:00p) /1-2 Evenings are required weekly until 8 pm.

ChristianaCare, CareVio Services is looking to hire a Registered Nurse (RN) Care Coordinator - Longitudinal.
CareVio Registered Nurse (RN) Care Coordinators connect with people by phone, video, in-person visits, secure texting, and e-mail to help them manage their health care. Our focus is on delivering healthcare through a combination of advanced technology and the human touch.
RN Care Coordinator Coordinates the utilization of healthcare resources, including transition planning, and facilitates the achievement of clinical, quality, financial, and member satisfaction goals.
RN Care Coordinator is responsible for ongoing contact with member and multidisciplinary care teams including CareVio case managers, Providers, Payers, Inpatient Case Managers, and alternate level of care settings staff.
Longitudinal – Practice/Provider Care Coordinator Role:
Strong knowledge base of co-morbid conditions and symptom management, including Diabetes and CardioPulmonary conditions
The position is hybrid supporting in office and remote work.
Onsite within practices as required per assignment.
Presentation and/or facilitation of Population Health Learning Collaborative meetings monthly at each assigned practice
Review and preparation of Provider Practice Weekly Huddles, if applicable
1-2 Evenings are required weekly until 8:00 pm

Experience & Education Requirements:
DE RN License or RN Compact License required.
BSN required or commitment to obtaining BSN within three years of hire date.
2-3 years’ experience in Utilization Management or Case Management required.
1-2 years of acute care in a hospital setting is preferred.
Basic Life Support (BLS) certification is required.
Case Management Certification is preferred through the Commission for Case Management (CCM) within 18 months of eligibility.
Advanced Certification is recommended.

Knowledge of resources and Nursing Theory and understanding and acceptance of Nursing Code of Ethics.

Ability to exercise clinical judgment.
