Job Description:
Assists the RN Care Coordinator in the follow up regarding information in the care plan, service authorizations, and the submission of data in to the multiple different health plan sites along with the Department of Health and Human Services sites. As well as assists in the reinforcement of the goals, completed by the RN Care Coordinator, and the collaboration of care with the care team for that member.

Key Responsibilities:
- Provides communication with patients, care teams, and other stakeholders to ensure that all relevant information is available to provide the best possible quality of care
- Assists the RN Care Coordinator with scheduling appointments, coordinating care, providing follow-up services to patients, submits data to health plans, and any other duties as assigned
- Tracks and maintains patient records and insurance information
- Provides administrative support including data entry, documentation in the Electronic Medical Record, and other duties as required
- Collaborates with other departments to ensure the efficient delivery of care while demonstrating a strong commitment to providing compassionate and quality care
Required Qualifications:
- Medical Secretary, Health Unit Coordinator, or similar program
- Minimum of 2 years’ experience in a medical setting if no education meets the Education Requirements listed

Compensation Range: $17.27 - $25.74 / hour
