Registered Dietician
Parma, OH- Mount Alverna Village
Full-Time- Exempt
The Registered Dietician is responsible for Nutrition Assessment, Care Planning process, Care Conference process, Resident visitation and interview, Meal observation, Conduct Quality Assurance audits. Participate in the Interdisciplinary team at Resident Care Conference as needed. Become familiar and update on the rules and regulations for Long Term Care Facilities. Assist the supervisor in any other Nutrition related activities. Fill in, in the absence of the supervisor and perform any task required by supervisor.
Specific Duties and Responsibilities:
- Will fill out and complete all required paperwork according to Professional and Federal guidelines in accordance to the facility policies within the specified time frame, (MDS 3.0, Nutrition Assessment, Medication sheet, Skin Sheet HCFA, Braden Scale, CAA’s, Clinical Quality Assurance, Care Plan, Monthly Accumulative weight sheet, Interdepartmental Referral, Dietary Change slip, Physician Order others as needed.
- Meets with all new residents, will visit and observe resident a minimum of once every 90days or on as needed basis. Weekly conduct meal observations on various units. Observe specific residents as well as the environment and compliance with 42CFR 483.35, Support (a) Staffing – Requirements for Long Term Care Facilities.
- Maintain a high standard of professionalism with the Dining Services associates, Physicians, and other direct and indirect care associates, provide diet information to the resident, family members, Sister, Wellness program and associates as needed
- Assist supervisor in planning and executing Nutrition update, In-service/Training and National Nutrition Month activities within the facility, Nurse Aid Training Program, Senior Health & Fitness Day and Training of Dietetic Interns.
- Assist in designing and implementing new or revised Nutrition assessments or interventions to ensure quality Nutritional support.
- Represent Dining Services at the scheduled Resident Care Conferences according to dates and times identified by the Care Conference schedule and supervisor, as needed.
- Incidental documentation regarding an improvement, weight loss or gain, decline, needed change, visit, discussion with other disciplines or family regarding the Nutrition interventions/status of the resident and any Discharge plan. Inform supervisor about incidental and significant changes with Nutrition interventions/status of the residents.
- Attend morning report when available, on a weekly basis obtain updates on residents in conjunction with current practice within the facility.
- Fill in, in the absence of the Supervisor (i.e. Vacation, seminars, workshops, illness.)
Qualifications/Requirements:
- Graduated from an accredited college obtaining a 4 year College Degree BS/BA in Nutrition/Dietetics with a completed approved internship or equivalent and successfully passed the Registration Exam from the Academy of Nutrition and Dietetics
- Completion of internship as per required agency and successfully passed the Registration Exam from the Academy of Nutrition and Dietetics.
- Previous experience in Clinical nutrition in either the Acute setting or Long Term Care.
- Academy of Nutrition and Dietetics, Commission on Dietetic Registration Licensed in the State of Ohio as a Dietitian or Currently meeting the Registration requirement, Active License in the State of Ohio
