The Case Manager coordinates all services necessary to maximize consumer recovery and independence for individuals with mental illness including implementing strategies for obtaining services and supports that are goal-oriented and individualized. This opportunity is located at our Saginaw Crisis Residential Unit.
The work hours include alternating weekends. The program is intended to be a short-term alternative for individuals at risk of admission into a psychiatric inpatient hospital but who can be appropriately served in a setting less intensive than a hospital.

How you will change lives Case Management - Ensure consumer involvement in the Person Centered Planning Process (PCP). - Facilitate the implementation of Individual Plan of Service. - Perform comprehensive, ongoing psychosocial assessments. - Participate in legal proceedings including petitioning and testifying in clinical emergencies. - Meet with assigned persons-served face-to-face as deemed medically necessary. Coordination of Care - Facilitate the coordination of consumer intakes, discharges, and transfers. - Communicate with all service providers in Treatment Plan. - Facilitate hospitalizations, attend hospital meetings and coordinate with hospital social worker. - Coordinate and monitor activities such as a job, school, volunteerism, and family caretaking. - Provide referral to appropriate community resources. About you - You are passionate about serving others. - You are helpful and professional with internal and external customers. - You enjoy working independently and as part of a team. - You act as a positive role model and coach for others. - Able to work 3:00 pm - 11:00 PM with alternating weekends. We are Hopeful you have - Bachelor’s degree in a human services (Social Work, Psychology, Sociology, Criminal Justice) field from an accredited institution required. - Prior case management experience in a mental health or rehabilitation setting preferred. - Valid Michigan Driver’s License.
