Job Description:
Plan, organize, control, and supervise clinical activities in the hospital pharmacy according to hospital policies, standards of practice of the profession, and State and Federal laws. Recommends innovation in the practice and function of the hospital pharmacy to the Director of Pharmacy and carries out mutually agreed upon programs. Responsible for staff education, residency program and ongoing programs related to drug therapy, such as anticoagulation program, drug utilization review, medication use and adverse drug reaction monitoring. Serves on committees as deemed necessary. Work Experience:Minimum five years clinical pharmacy experience.Minimum two years experiences as a staff pharmacist or residency in an ASHP accredited program.Minimum one-year experiences as a supervisory pharmacist involved with program development.
Education Qualifications:
Graduate of a college of pharmacy accredited by the American Council on Pharmaceutical Education (BS or PharmD degree required)
Licensure/Certification Qualifications:
Licensed as a Registered Pharmacist by the Board of Pharmacy of the state in which they will be working (Minnesota, Wisconsin, North Dakota.
If currently registered as a Pharmacist in another state, must be registered with the Board of Pharmacy of the state in which they will be working (Minnesota, Wisconsin, North Dakota) as a Pharmacist-Intern and actively pursuing licensure as a Registered Pharmacist, obtaining licensure within 90 days of hire.
Ongoing continuing education and license renewal as required by the state Board of Pharmacy.
