Job Summary and Qualifications
As an Advanced Care Planning Coordinator, your primary responsibility is to provide consultation regarding patient/family palliative care issues and serve as an education liaison to patients, families, and the interdisciplinary team. In addition to these responsibilities, you will:
- Maintain knowledge base with and awareness of current trends that influence health care practice in areas of palliative care
- Demonstrate the ability to participate in performance improvement activities and correct systems and clinical issues that affect patient outcomes
- Serve as a clinical liaison in area of specialty
- Demonstrate a knowledge base of various financial classes and regulations of their respective institutions to facilitate patients through the continuum of care
- Demonstrate an active role in discharge planning
- Other duties as assigned
What qualifications you will need:
- Graduate of an accredited school of nursing required, BSN preferred
- Licensed a Registered Nurse in the Commonwealth of Virginia or has a Compact license
- BLS required
- At least two years’ experience as a RN is required
- Knowledge of palliative care services is highly preferred
- Experience in a specialty area with case management and/or oncology/hospice nursing experience preferred
