Job Summary
The Cardiac Nurse Navigator (RN) serves as a member of the multidisciplinary care team with a focus on individualized care of cardiac surgery patients. Provides ongoing patient/family education and supports the coordination of care, as directed by the cardiac surgeon and cardiac surgery provider team. Key Responsibilities include but are not limited to:

  • Provide pre-op education to cardiac surgery patients/families primarily in the ambulatory setting with a focus on providing excellent patient/family experience.
  • Collaborate with other health care providers (Vascular Surgery, Cardiothoracic Surgery, Heart Failure, Interventional Cardiology, Hospitalists, Nursing, Social Workers, Case Coordinators, Pharmacy, Rehabilitation Medicine, Palliative Care, etc.) to decrease in-patient length of stay and readmissions of cardiac surgery patients by facilitating a smooth transfer of care to and from the community.
  • Identify specific patient and/or family psychosocial, interpersonal and clinical care needs during the preop, intraop and post op cardiac surgery period. Develops plan of care to address these needs and potential barriers.
  • Facilitate post-op ambulatory care of cardiac surgery patients and provide post-op education as needed.
  • Act as the subject matter expert and provides ongoing education for the entire cardiac surgery team regarding patient resources i.e. tobacco cessation programs, home care options, cardiac rehab, transportation options, etc.
  • In collaboration with the clinical cardiac surgery team, participates and facilitates ongoing performance improvement initiatives, committees, etc.
  • Collaborate with key stakeholders to ensure that cardiac surgery patient education is up-to-date and pertinent.
  • Organize and facilitate multidisciplinary team meetings for individualized patient-centered care when requested by clinical team.

Qualifications

Education:

  • BSN required Master's Degree in Nursing preferred

Experience:

  • 5 years nursing experience required
  • 3 years clinical in-patient OR/cardiac surgery/Critical Care Nursing experience preferred
  • Certification in a specialty area preferred
  • Fluent in Spanish highly preferred

Licensure:

  • Connecticut Registered Nurse license
