DescriptionSummary:
This position is responsible for providing care to assigned patients that meets their physical, psychological, social, cultural, spiritual, and developmental needs. Functions as a contributing member of the health care team.
Responsibilities:
- Assists the team leader in assessing the status of patients with simple health problems on admissions and on a continuing basis reporting changes in health status to the RN and / or Physician.
- Contributes information to the plan of care.
- Implements the plan of care, under the direction of the RN and / or Physician and within the scope of practice, to meet the basic needs of assigned patients and families.
- Documents data for assessments, interventions and effectiveness of care provided in the patients medical record. Findings are reported to the RN and / or Physician.
- Participates in patient education activities as directed by the RN and / or Physician.
- Assists in the preparation of the patient and family for discharge under the direction of the RN and / or Physician.
- Administers medications to assigned patients in accordance with clinical specific standards.
- Responds to emergencies.
- Identifies safety hazards and reports them
- Demonstrates competence to perform assigned patient care responsibilities in a manner that meets the age specific and developmental needs of patients served by the department.
- Appropriately adapts assigned patient assessment, treatment, and/or care methods to accommodate the unique physical, psychosocial, cultural, spiritual, age specific and other developmental needs of each patient served.
- Demonstrates adherence to the CORE values of Santa Rosa Health Care.
- Perform other duties as assigned.
Requirements:
Education:
- High School Diploma or GED Required
- Graduate from an accredited school of vocational nursing
Experience:
- Experience in clinical setting preferred
Licenses, Recreations, or Certifications:
- Licensed as a LVN in Texas BLS Certification required
