The Registered Nurse Clinical Educator II (RNCE) will assess, plan, coordinate, implement and evaluate the clinical opportunities and strategies for improving delivery of diabetic patient care and staff education regarding such. The RNCE will act as a liaison with all multi-disciplinary departments and services concerning diabetes education. The RNCE will develop and provide diabetes education for patients and their families on an inpatient and outpatient basis as directed.
CHRISTUS Santa Rosa Hospital - Medical Center (CSRH-MC), located in the South Texas Medical Center, provides a high-level of care through compassionate hands, experience physicians, and advanced technology, including the da Vinci Surgical Robot System and the Mako Robotic-Arm Assisted System. Services at the 178- bed facility include Acute Care of the Elderly Unit, cardiology, diabetes care, emergency care, general surgery, kidney transplantation at the CHRISTUS Transplant Institute, orthopedics, outpatient rehabilitation and aquatic therapy, Senior Behavioral Health Center, urology, and the Wound Care and Hyperbaric Center.
Responsibilities:
- Provides evidence-based, safe, effective and appropriate diabetic consultation for populations served.
- Facilitates the delivery of knowledge and supporting glycemic management efforts within CHRISTUS Santa Rosa facilities.
- Educates and coaches nursing and other clinical staff to better manage diabetes care for their diabetic patients.
- Conducts inpatient clinical rounding on both patients and clinical staff.
- Provides subject matter clinical consultation for high risk, complicated diabetic patients.
- Provides outpatient diabetes education programs as needed/directed.
- Develops and maintains clinical protocols, policies, and patient education for diabetes care and glycemic management.
- Works collaboratively with interdisciplinary team to promote, build and sustain a high quality diabetes education service.
- Assists with improving quality outcome measures and enhanced patient satisfaction.
- Researches data and compiles reports regarding the efficacy of the diabetes education program.
- Acts as a community resource for diabetes education.
- Appropriately adapts assigned patient assessment, treatment and/or care methods to accommodate the unique physical, psychosocial, cultural, age-specific and other developmental needs of each patient served.
- Performs all duties in a manner that protects the confidentiality of patients and does not solicit or disclose any confidential information unless it is necessary in the performance of assigned job duties.
- Demonstrates good stewardship in proper use and maintenance of equipment and supplies.
- Performs other duties as assigned.
Requirements:
Education:
- Graduate of an accredited RN program required
- BSN preferred
- Bilingual (English/Spanish) preferred
Experience:
- Minimum of three years clinical RN experience required
- Minimum of three years experience managing the care of patients with both Type 1 and Type 2 Diabetes Familiarity with insulin pumps preferred
Licenses, Registrations, or Certifications:
- Current Texas RN license required
- Certified Diabetic Educator (CDE) preferred. If not CDE at time of hire, successful completion of CDE exam within 2 years of hire/transfer date is required to remain in position
- Current BLS certification required – American Heart Association
