Your Role
The Clinical Access Program’s Initial Health Appointment Outreach Team ensures that all newly enrolled Medi-Cal members have an IHA visit with their PCP within 120 days of enrollment. The Clinical Services Coordinator will report to the Clinical Training and Auditing Supervisor. In this role you will be responsible for making outreach calls to Medi-Cal members on a daily basis and maintaining documentation of this outreach
Your Work
In this role, you will:
- Perform daily outreach to Medi-Cal members to help coordinate Initial Health Appointments
- Be responsible with handling Personal Health Information (PHI)
- Document calls and call outcomes in spreadsheets and/or dashboards
- Research member eligibility in Sales Force (Care Connect) and FACETS
- Assist with regulatory audits
- Interact with provider offices for both Los Angeles and San Diego counties
- Participate in Team and Department meetings
- Use phrases that begin with singular verbs (e.g. “Manage critical programs including new hire onboarding and people leader training”)
- Avoid periods for all bullet points

Your Knowledge and Experience
- A minimum of a high school diploma
- Requires at least 3 years of prior relevant experience
- Bilingual Spanish Prefered
- Expertise in Microsoft Office (Word, Excel, Outlook), Sales Force (Care Connect), FACETS
- Capable of working independently and as part of a team
- Excellent written and verbal communication skills
- Health Plan, IPA, Public Health, Quality Improvement experience desired
Pay
The pay range for this role is: $ 20.70 to $ 28.98 for California.
