Cleveland Clinic’s Social Work team provides caring, patient and family-focused social work services to meet basic human needs, enable clients to reach their optimal level of psychosocial functioning and enhance well-being.This position is critical to the delivery of quality psychosocial care and management of patients receiving cancer care at our regional sites.

A caregiver in this position works the day shift from 8:00AM to 5:00PM.


A caregiver who excels in this role will:

  • Work collaboratively with healthcare teams to assess and plan interventions to help patients cope with social, emotional, financial and environmental problems.

  • Perform psychosocial assessments and provide interventions to assist patients and families. This may include counseling, education, advocacy, referrals, conflict resolution, crisis intervention, problem solving and coordinating family/team meetings.

  • Provide and/or document treatment recommendations, professional recommendations, care coordination interventions and treatment plans.

  • Engage patients and families in adhering to the treatment plan by reducing or eliminating barriers.

  • Maintain a network of resources for consultation and connect patients with community resources.

  • Supervise social work interns and assist with training new caregivers.

  • Educate other caregivers about the impact of social problems on health outcomes and the effectiveness of social interventions in improving outcomes.


Minimum qualifications for the ideal future caregiver include:

  • Master’s degree in social work from a CSWE accredited program.

  • State Licensed Social Worker (LSW).

  • Two years of experience in a hospital or healthcare setting.

  • Licensed Independent Social Worker (LISW) within five years of hire.

Physical Requirements:

  • Manual dexterity to operate office equipment.

  • May require periods of sitting, standing and the ability to walk to various locations throughout the Foundation to attend meetings.

  • Must have normal or correction vision

  • Must be able to clearly communicate verbally by phone.
