A Brief OverviewResponsible to obtain medical history on pre-surgical and clinical radiology specials patients and provide education. Assists in completing of charts for surgical procedures, radiology specials procedures and other areas as directed. Responsible for applicable assessment, triage, planning, education on interventions/options, education and evaluation of outcomes. This role provides support and follow-up calls per outlined protocols, facilitating referrals to local physicians and community services and ensures hand-offs between services are completed.
What you will do
- Collects patient health data.
- Documents nursing history for assigned patients via phone interviews. Initiates anesthesia record and home medication record.
- Assures appropriate in hospital referrals are completed to provide optimal patient care (i.e., pastoral care, anesthesia, patient representative).
- Explains procedures and treatments to patient to gain cooperation, understanding, and decrease apprehension.
- Provides general surgical/clinical education and instructions to all interviewed surgical patients, responds to patient questions and refers patients.
- Decisions and actions on behalf of clients are determined in a manner consisted with the ANA’s Code of Ethics.
- Collaborates with the patient, significant others, and healthcare providers in providing patient care.
- Utilizes research/evidence-based finding in practice if appropriate.
- Participates in department or unit quality improvement activities.
- Knowledge and skills sufficient to integrate the use of healthcare technology and/or information management systems into the preadmission nursing role.
