A Brief Overview

Provides support to Avera@Home Home Health and Hospice Agencies for organization-wide initiatives and agency-specific quality assurance/process improvement (QAPI) projects. Project deliverables and outcomes are shared between the agency manager and the Quality Partner and will include development and implementation of the agency-specific quality plan, staff orientation/education, auditing and hardwiring of processes to support projects. The Quality Partner will support the Agency Manager with a lean, compliant and standardized environment which follows the Avera@Home Care Delivery standards.

What you will do

  • Evaluates all available Quality/Compliance data for each agency to identify opportunity for improvement, shares findings with the agency leadership monthly to assist them with determining needed areas of improvement and prioritizing improvement efforts.
  • Partners and supports Avera@Home agencies and works 1:1 with them, utilizing a consultative approach to achieve successful outcomes on: Quality Assurance/Process Improvement (QAPI), Value Based Purchasing (VBP), Home Health Compare, Care Delivery, and Operational Projects.
  • Assists agencies in maintaining a CMS/ State Licensure survey ready environment.
  • Assures agency staff have received appropriate education by collaborating with Avera@Home Leadership for input on educational needs related to Avera@Home staff.
  • Coordinates the development, planning, implementation, and evaluation of clinical staff education and orientation programs.
  • Develops, implements, and maintains yearly competency programs for all clinical staff.
  • Completes a yearly evaluation of the education program for Avera@Home.
  • Serves as a resource in the development, implementation and evaluation of policies, practices, and standards of care.
  • Administrator of process improvement and educational tools.

Required Education, License/Certification, or Work Experience:

  • Registered Nurse (RN) - Board of Nursing An active license in the state of practice. Upon Hire

  • 4-6 years Related experience and/or training
