Position Summary

We’re seeking an experienced leader to work collaboratively with the Area Manager and Regional Director overseeing the Acute Care team at The Hospital of Central Connecticut in New Britain, CT.

Overview:

In addition to direct patient care responsibilities, the Site Supervisor supervises the operational activities of the Acute Care team and collaborates with Rehab and Hospital Leadership on the management of patient care, day-to-day operations and department personnel. They are responsible for staff development, hiring, supervising and evaluating team members.

The Site Supervisor assists with supply management, policies and procedures, patient scheduling, patient and staff safety, operational monitoring, regulatory compliance, employee engagement and the overall timeliness and quality of services provided to customers.

The Site Supervisor participates in multiple committees across the hospital, representing Rehab. They are responsible for metrics reporting and dashboards, as well as referral management, PSAG, and other patient care and safety huddles. Manages all DPH and Joint Commission audits.

Additionally, approximately 50% of time is spent in a direct patient care capacity.

Supervisory Responsibilities

·Responsible for coaching, training, onboarding, employee engagement, staff rounding, timekeeping, onboarding new staff and performance evaluations. Leads daily management huddle boards.

·Delegates patient care tasks. Provides clinical supervision in area of specialty as required.

·Actively participates in enhancing patient experience, quality control, and process improvement initiatives.Ensures regulatory compliance and assists with incident resolution, policies/procedures, and emergency preparedness.

·Identifies opportunities for improvement, recommends process improvements utilizing operating model, and takes necessary steps to improve processes

·Responsible for managing all DPH and Joint Commission audits. Makes sure all staff are compliant with education, all files are reviewed, responds to all inquiries.

Clinical Responsibilities:

·Completes all documentation including but not limited to: evaluation, care plan and goals, patient status and progress, visits, patient discharge summary, and patient care transition plan and summary.

·Participates in patient transition planning and/or patient discharge planning including recommendations to community resources to meet patient needs throughout the continuum of care. Helps to coordinate patient care with other care providers such as Rehab Therapists, Nurses, Social Workers, and other community resources as related to care transition planning.

Qualifications

·Physical Therapist (PT) or Occupational Therapist (OT) license in the state of CT

·Bachelors, Masters or Doctoral Degree in Physical Therapy or Occupational Therapy

·Minimum 5 years’ clinical experience required

·Experience as a clinical instructor or mentor required

·Strong verbal and written communication skills required

·Computer proficiency in Microsoft Word, Excel, PowerPoint as well as electronic health records and other software applications

·Ability to solve problems and work independently with minimal direction

·Teamwork and mentoring skills

·History of strong leadership, communication, and clinical skills
