LPN
JOB SUMMARY
Under the direction and supervision of the Clinic Services Supervisor, uses and practices basic assessment (data collection), participates in planning, executes, interventions in accordance with the care plan and contributes to the evaluation of individualized interventions related to the care plan. Administers medications, implements primary care orders, and evaluates participant complaints physically and/or by telephone. Provides education to the PACE organization, participant, and caregiver in regards to care plan, nursing treatments or medications. Communicates participant’s condition to primary care givers, physician and multidisciplinary team.
REQUIREMENTS
- Graduate of an accredited school of nursing.
- Current LPN license in good standing to practice in the required state.
- IV Certification preferred.
- Experience with paperless documentation preferred.
- 1 year of experience working with a frail or elderly population preferred. If this is not present, training on working with a frail or elderly population will be provided upon hiring.
- Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.
- Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.
- Pass a comprehensive criminal background check that may include, but is not limited to, federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen.
SKILLS AND ABILITIES
- Broad knowledge base of health care problems of the frail elderly.
- Knowledge of medical equipment and instruments.
- Knowledge of common safety hazards and precautions to establish a safe work environment.
- Skilled in identifying problems and recommending solutions.
- Skilled in preparing and maintaining records and writing reports, and responding to correspondence.
- Skilled in developing and maintaining department quality assurance.
- Skilled in establishing and maintaining effective working relationships with participants, medical staff, staff members and family caregivers.
- Ability to react calmly and effectively in emergency situations.
- Knowledge of medications used in geriatrics and adverse reactions.
- Skilled in wound care.
