Job Description Summary:
Join the OhioHealth team as a Physical Therapy Assistant! Experience one on one patient care, a flexible schedule, and a great benefits package. Home health PTAs work with an interdisciplinary team to care for patients in the home setting. Join our amazing home health team where you have autonomy, a reasonable and fair weekend rotation, a flexible schedule, and limited on-call. Home health PTAs provide skilled care to patients to achieve outcomes in collaboration with the patient’s provider.
Responsibilities And Duties:
Driving is an essential function of the job.
90% - Direct Patient Care – Evaluates and treats patients after physician referral.
Administers portions of the assessment that is assigned by the Physical Therapist (PT) in which Physical Therapy Assistant (PTA) has demonstrated to be competent.
Assists in reassessment process, reporting patient performance to the PT.
Assists PT in implementation of the patients plan of care based on occupational performance frames of reference(s): bio-mechanical, rehabilitation, sensorimotor.
Attends multidisciplinary patient care meetings.
Plans and helps coordinate discharge plan with multidisciplinary team.
Educates patient and caregiver/family.
Recommends Medical Equipment required for discharge.
Provides information to Physicians, Nurses, Care Managers, Social Workers, and other team members.

Documents evaluation performance directed and interpreted by the PT, progress towards goals, discharge plan/continuity of care for the multidisciplinary team, and other involved medical and insurance parties.
All documentation to be co-signed by the supervising PT.
5% - Indirect Department Functions.
Delegates assignments to Rehab Technicians.
Remains involved in department process improvements utilization review and quality assurance program.
Educates others about Physical Therapy.
Takes active role in orienting new staff members.
Assists in clinical instruction of PTA students.
Educates other disciplines and medical/hospital personnel on PT purpose, goals and treatment policies/procedures.
Assumes other duties delegated by management.

Minimum Qualifications:
Associate's Degree (Required)BLS - Basic Life Support - American Heart Association, DL-HC - Driver's License only if in a Home Care department - Department of Motor Vehicles, LIC - Licensed in Field - State of Ohio, PTA - Physical Therapist Assistant - American Board of Physical Therapy Specialties
Additional Job Description:

- Experience in an acute, rehabilitation or home health setting
- Basic personal computer skills
