Overview:
We are looking for a Care Manager Enhanced Palliative Care (EPC) to join our team and report directly to the AVP Enhanced Palliative Care Partnerships. This position is responsible for managing all day to day business affairs and other operational activities related to the administration of the Enhanced Palliative Care department.
- Coordinate and direct all day-to-day operational activities related to the administration of the Enhanced Palliative Care department
- Process referrals by obtaining sufficient admission and insurance information in order to ensure appropriate and timely admission of patients
- Support care plan development and implementation for palliative care from the initial assessment through discharge. Assessment at minimum should include: disease-specific changes, decline or change in function, nutrition and/or cognition, inadequately controlled pain or other distressing symptoms, falls, infections or similar events, the impact of the disease burden on the patient’s condition, caregiver and/or family’s quality of life
- Facilitate prescription transmission to pharmacy including controlled substances to the extent delegated and licensed in accordance with state law. Receive laboratory results and other diagnostic tests, provide results to nurse practitioner in a timely manner. Place requests for ordered durable medical equipment as medically necessary.
- Collaborate on development and modifications to the plan of care, individualized for each patient in consultation with the patient, family, and other members of the IDT to include the PCP
About You:
- 3 years healthcare delivery or related experience
- 2 years of prior skilled Registered Nurse experience
- Active acute unencumbered state RN license
- Ability and interest to provide direct patient care in homes and facilities
- Demonstrated experience developing and maintaining rapport with patients and families, and ability to work well with an interdisciplinary team
- CPR certification, copy of TB results
- Current driver license and auto liability coverage
- Strong knowledge of medical terminology and a customer service focus
- Experience in a role related to direct to customer interaction and engagement preferred
- Effective data entry and word processing, problem-solving, human relations, and oral/written communications skills
- Experience with Computer applications and Electronic Medical Records
- Some travel may be required as requested
