Must assess, plan, implement and evaluate patients needs and care. Performs duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.Qualifications:Education: Graduate of an accredited practical nursing program; Consideration will be given for comparable education experience and successful licensure.Licensure: Current LPN license in the state of practice. Must be a licensed driver.Certification/Registration: Possesses and maintains current CPR certification.Other skills & knowledge:Complies with accepted professional standards and principles. Understands Hospice philosophy; issues related to death/dying and grief.Complies with Unity Health Hospice Health Status requirements. Has ability to provide good written documentation in a timely manner.Is flexible and cooperative in fulfilling role obligation and has ability to work within an interdisciplinary team.Employee has significant impact on patient welfare.
