Ensure that documentation and maintenance of client chart meets agency, state, federal, etc. audit standards. Collaborate with internal (i.e. medical, psychiatric, clinical, case management, peers, etc.) and external (i.e. referral agencies, family, friends, etc.) treatment team members and/or supports around a client’s treatment plan goals, treatment, and treatment progress. This clinician will enrich the team’s diverse experience by broadening the range of ways of understanding and engaging with our communities through culturally responsive service to others.Residential (Garcia House) - a specialty co-occurring residential Circle Program providing comprehensive ASAM 3.7 and 3.5 residential treatment to individuals with co-occurring substance use and mental health disorders.Essential Duties:Direct client care to include care coordination, behavioral health related screenings and assessments, individual and group therapy, brief therapy, case management, and related clinical services.Serves as a clinical resource person to building site(s), clinical staff, case managers, medical providers and collaterals.Knowledge of professional policies and procedures.Collaborates with other staff in providing, planning, developing, implementing, and evaluating clinical services, including collaborating with Clinical Managers and Program Supervisors.Performs behavioral health screenings and assessments within the medical clinic setting, to include review of treatment options and referral information.Provides on going care coordination within SummitStone, community agencies and resources.Provides crisis intervention to include crisis assessment and de-escalation.Provides onsite collaboration with medical providers to provide behavioral health assessments for those clients identified with behavioral health needs.
