In addition, the Physical Therapist instructs, educates and trains patients and caregivers in those skills and functions essential for promoting independence and productivity, in order to diminish or correct pathology. They direct activities to promote and maintain health. This position provides a flexible schedule and work environment in various settings.1. As necessary or applicable, supervision of Physical Therapy Assistants, temporary licensees, students and support personnel in accordance with state licensure requirements and professional standards.2. Responsible for adherence to department and patient treatment schedule, timely clinical and billing documentation requirements as well as work related meetings.3. Attends and contributes to patient care, staffing conferences and other related meetings.4. Makes referrals to other disciplines and agencies in order to address the comprehensive needs of the patient.5. Puts patient service first: ensures that patients and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals' needs and rights.11.Screens, examines and evaluates patients, including history, systems review, and application of appropriate tests and measures, synthesis information and interprets examination findings in order to establish a diagnosis, identify impairments, determine the predicted level of improvement and the time required to achieve it, identify precautions/contraindications and design and plan of care. Develops appropriate treatment goals and methods in collaboration with the patient and caregivers, implements the physical therapy treatment plan, and completes all related documentation and record keeping regarding these services. Performs routine reexamination as needed/required to modify/progress plan of treatment.16. Incorporates health and wellness programming into treatment plans for patients.17. Performs home and community mobility assessments and provides recommendations for safe mobility in the home and community space.18. Performs other related duties as required. Qualifications: 1. Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility; and2. They must have a Master's degree in Physical Therapy; or3. This is currently a part time (24 hour) position with the potential to temporarily increase to more hours during the week for coverage of other clinicians. Our Outpatient Rehab Clinic in Westport, CT is our flagship location within Fairfield County. It is connected to the Ayer Neuroscience Spine Wellness center and is part of a multidisciplinary team of medical, rehabilitation, and wellness professionals dedicated to improving the health of all patients. This therapist will collaborate with professionals throughout the facility who provide rehabilitation, pain management, physical medicine, integrative medicine, fitness, nutrition, and behavioral health services to improve surgical and non-surgical outcomes. All of our clinics have ample private treatment spaces and state-of-the-art gyms to support our patient’s recovery. Specialty Programs in this clinic include Balance & Falls (Safe Steps), Cancer Rehabilitation, Complex Spine, Concussion, Dry Needling, Gender-Affirming Rehab, Headache Management, Chronic Pain Management, Pelvic Health, Sports Medicine, Thoracic Outlet and Vestibular Therapy. The Physical Therapist (PT) independently evaluates, determines treatment strategies, delivers and/or delegates services that identify, prevent, remediate, and rehabilitate acute or prolonged physical dysfunction or pain, with emphasis on physical dysfunction. This role encompasses evaluation skills and techniques, analysis of patients, and therapeutic application of physical and chemical agents, exercise and other procedures to maximize the patient's functional independence. Evaluation involves strength, motor control, joint mobility, soft tissue integrity, pain, balance, coordination, posture, cardiopulmonary function, endurance, gait and functional abilities in activities of daily living. Responsibilities: Upon referral, the Physical Therapist (PT) performs thorough examination and evaluation of patient’s level of function, includingstrength, movement, motor control, joint mobility, soft tissue integrity, pain, balance, coordination, posture, cardiopulmonary function by assessing and monitoring vitals, endurance, gait and functional abilities in activities of daily living. The PT recognizes physiological, psychological, and age-related changes in the patient.The Physical Therapist (PT) develops an individualized patient-centered treatment plan of care to promote optimum movement function and safety.The Physical Therapist (PT) implements, delivers and delegates patient care by utilizing appropriate interventions and clinical decision making including but not limited to modalities such as heat, cold, light, water, sound, electricity, mobilization and therapeutic exercise.The Physical Therapist (PT) helps to coordinate patient care with other care providers such as Rehab Therapists, Nurses, Social Workers andparticipates in patient transition planning and/or patient discharge planning including recommendations to community resources to meet patient needs throughout the continuum of care.
