You will build relationships with your patients, focus on preventive care and chronic conditions, and coordinate patient care needs as part of the interdisciplinary Advanced Primary Care team. In addition to rooming and managing clinic flow for the provider, you will provide immunizations and medications, perform procedures like wound care, POCT and lab draws, as well as assist providers with procedures. Provide patient-centered education of disease processes and wellness opportunitiesUse assessment and analysis skills in delivering medical regimens as directed by physicians and APPs.Assist patients and families toward self-management of care including goal setting for management of chronic disease, improving medication adherence, creating a plan for behavioral change and coordinating community resources via nurse visit, telephone management, or other contacts, as neededDelegation of selected patient care functions to others in accordance with their education, credentials, and demonstrated competence.Serve as resource for staff, colleagues, physicians and advanced practice providers to positively impact CMS core measures and National Patient Safety Goals.Use critical thinking skills in making decisions regarding patient care, which includes knowledge and integration of relevant standards, resources, and data. Advocate for patients and families, ensuring sensitivity to cultural and ethnic practices and beliefs/for ethical and holistic care promoting the autonomy, dignity, and health of patients from diverse ethnic and social backgrounds.QualificationsBachelor's degree in Nursing, or obtained within five (5) years from the initial date of hire; RN with 15+ years of service or internal candidates hired before January 1, 2021 may be considered in satisfaction of this requirement in lieu of an education agreementMaster’s degree in nursing preferredCurrent Registered Nurse licenses in Washington State or Multistate license endorsementCurrent BLS CertificationThree (3) years of recent clinical hospital/ambulatory experienceKnowledge of care continuum (inpatient, ambulatory, post-acute, etc.) preferredCertification as Certified Case Manager (CCMC), Accredited Case Manager (ACMA), Care Coordination and Transition Management (AAACN), Ambulatory Care Nursing (AAACN), or similar required or obtained within 3 years of hire (Employees hired prior to 8/1/2023 will have this requirement waived)Our ValuesAs a MultiCare employee, we'll rely on you to reflect our core values of Respect, Integrity, Stewardship, Excellence, Collaboration, Kindness and Joy. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $96,689.00 - $161,160.00 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education.
