The Social Worker for Community Health Initiatives provides professional services to clients and their families eligible for the Healthy Beginnings Services in order to meet identified psychosocial, emotional, financial and environmental needs. This SW will also serve patients/families on board the Mobile Health Clinic performing Social Determinants of Health and providing appropriate referrals/resources. The social worker provides psychosocial assessments, supportive counseling, emergent crisis intervention appropriate to setting, financial resource information, environment enhancements, advance directive planning and referrals to community agencies for clients and their caregivers/families in the client's home, mobile health clinic (MHC), or other settings. The Exercise Physiologist monitors outpatient cardiac rehab and may perform a variety of non-invasive cardiovascular exams.Provides care including the development, implementation and evaluation of exercise programs for cardiac patients in phase 2 and 3 of cardiac rehabilitation.Works collaboratively with all members of the patient's care team to ensure services are consistent with post op and/or post hospitalization care.Evaluates and revises plan of care as needed in collaboration with RN and physician when necessary.Develops and advances the patient's exercise program based on the physiological and electrocardiographic response to exercise.Provides health education and teaching for patient and family.Complete required feedback and communication to patient and healthcare team including EMR documentation and reports concerning education, exercise progression, exercise treatment and monitored physiological and vital responses of the patient.Assists with scheduling and billing of cardiac rehab services.Precepts nursing, exercise physiology and nutrition students as assigned to the program.Schedules patient for cardiac rehab initial visitsConducts initial visit patient appointmentsPerforms patient heart disease risk factor assessmentsCoordinates patient care plans for outpatient cardiac rehab, phase II and phase IIICoordinates patient exercise class schedulesMonitors patients on telemetry during exercise sessionsLeads patients through exercise prescriptions and assists patient with exercise equipmentProvides education to patients to support risk factor modificationReports adverse events to ordering providersMonitors patients during exercise stress testsApplies ambulatory holter and event monitors
