The Home Care Field RN is a professional caregiver who is a member of the interdisciplinary team. Responsibilities include the coordination and provision of direct and indirect patient care using the nursing process to meet the physical, psychosocial, environmental, and spiritual needs of Carle Home Services specific patient populations and families throughout the geographical area.Demonstrates understanding of Medicare Home Care benefits including covered services, recertifications, transfers, and non-recertifications. Follows regulatory requirements regarding documentation standards.Comprehends department specific indicators.Performs case management activities of assessment, coordination, planning, monitoring, implementation, and evaluation. Interacts with clients, caregivers and families to assess, plan care, arrange services, monitor, and provide support and education.Consistently completes all admission documentation within policy guidelines.Returns admission documents to office complete and in a timely manner (according to policy).Completes required OASIS/485 changes within 24 hours of receiving them.Completes visits notes and telephone conversations records and transmitted timely according to policy (includes physician orders, admission, routine, supervisory, telephone and discharge notes.)Ensures Physician Orders are written timely, corrections to care plans are entered and transmitted according to time line.Reviews verbal orders with MD office, written and evidenced as read-back, completed immediately and transmitted within time frame established by policy. The Surgical Assistant functions as an integral part of the perioperative team as the assistant to the surgeon. The duel modality CT and Xray technologist will cover our after hour on call needs for the Radiology department. The technologist will performed various computerized axial tomography and x-ray imaging procedures on various imaging equipment and possibly administering contrast material based on Carle policies and procedures.Performs computerized axial tomography procedures and assign invasive procedures by acquiring and processing patient exams in accordance with accreting bodies and / or manufacturer guidelines and as outlined in department protocols. This includes performing multiplanar reformats and /or 3D rendered images independently. Reviews cases with the Radiologist when appropriate. reviews labs and patient history to perform correct exam. Administers IV contrast, and oral/rectal contrast when appropriate. Recognizes and responds to any adverse reactions or allergic reactions. Screens the patient for possibility of pregnancy and performs pregnancy testing if applicable.Ensures patient, visitor and environmental safety. Reports safety events immediately including those that are a near miss.Assists in instruction of students and new staff as necessary. May perform duties of CT Lead or Imaging Coordinator in his/her absence.Meets productivity levels as established by department benchmarking. The Registered Radiologic Technologist is a professional caregiver who assumes responsibility of patients for a designated time frame and to provide care, specifically in the Heart & Vascular Institute. The patient population consists of those requiring an invasive cardiology or invasive radiology study. The behavioral health clinician, as part of the primary care treatment team, triages and manages patients with medical and behavioral health problems within the primary care setting. The behavioral health clinician provides quality clinical mental health consultation to providers and staff as requested. Additionally, the behavioral health clinician provides diagnostic mental health assessments, interventions, education, and referrals to further treatment to patients in the primary care setting.
