Position Highlights: CT7B Advanced CareNumber of beds on unit: 24Patient population (age, diagnosis, specifics about unit): The Advanced Care Unit is a 24-bed unit specializing in the care of acutely unstable patients that do not meet criteria for Intensive Care status but still require frequent, advanced assessments and interventions. Specific patient populations or conditions that require Advanced Care status include complex multi-trauma, uncompensated sepsis, continuous non-invasive ventilation, stroke, complicated withdrawal from alcohol or other recreational drugs, and DKA. The unit also includes an Epilepsy Monitoring Unit (EMU) where patients with seizure-like events can be safely taken off medication to purposefully induce seizure-like events and provide vital information to the Epilepsy Team about how to treat them.Nurse to Patient ratio: 1:3Department culture and resources: The Advanced Care Unit is a fast-pace, team-oriented unit. We work closely with our physician partners and ancillary team members to achieve the best outcomes for our patients. We have a robust Partnership Committee that meets monthly. Surgical patients on this unit includes trunk trauma patients, as well high acuity surgical advance care patients. We care for patients with surgical intervention needs from Oral Maxillary Facial Surgery (OMFS) traveling through to Colon Rectal Surgeries. Responsible for the oversight, coordination, and management of the functional and financial outcomes during acute illness requiring hospitalization for patients of the Carle Foundation Hospital. Ensures patients receive proactive initial assessment of needs, ongoing evaluations, and initiation of discharge planning while facilitating a safe and timely transition from the acute care/hospital setting to an appropriate level of care outside the hospital. Utilizes the five components of case management: assessment, coordination, monitoring, implementation, and evaluation. These patients are redirectable but need frequent rounding due to confusion, impulsiveness, or lack of understanding of their limitations.
