OverviewThe Transitional Care Clinic TCC) Nursing Coordinator maintains accountability for daily operations and delivery of patient care through supervising, leading and mentoring personnel in the TCC. Serves as a professional leader, role model, and resource to department staff and performs personnel actions, staff development and training, evaluation and counseling/coaching. This position will work closely with the physician dyad partner and other system leaders to develop and implement a comprehensive and cohesive model that optimizes coordination of care and care transition across the enterprise and region for high risk patients.
