Therapists will develop, document, and monitor treatment plans; monitor and document treatment services, interactions, and progress. Ensure that documentation and maintenance of client chart meets agency, state, federal, etc. audit standards. Collaborate with internal (i.e. medical, psychiatric, clinical, case management, peers, etc.) and external (i.e. referral agencies, family, friends, etc.) treatment team members and/or supports around a client’s treatment plan goals, treatment, and treatment progress. Coordinate with collaterals and/or partner agencies during and after a client’s treatment to ensure sustainable support and step-down services are in place. Members of these teams are an integral part of SummitStone and our Northern Colorado community, working closely with a myriad of other organizations to help ensure that special populations are served with unconditional positive regard. This clinician will enrich the team’s diverse experience by broadening the range of ways of understanding and engaging with our communities through culturally responsive service to others.Essential Duties:Direct client care includes care coordination, behavioral health related screenings and assessments, intake assessments, diagnosing, individual therapy, brief therapy, case management, referrals, and related clinical services.Serves as a clinical resource person to building site(s), clinical staff, case managers, medical providers and collaterals.Knowledge of professional policies and procedures.Collaborate with other staff and supervisors in providing, planning, developing, implementing, and evaluating clinical services.Provides crisis intervention to include crisis assessment and de-escalation.Multidisciplinary coordination and collaboration approach internal as well as external providers and supportive services to deliver needed services to clients.Perform Clinical documentation and other duties as assigned in a timely manner.Supports and fosters team values with peers and clinic staff, to provide support and remove barriers.Maintains superior customer service for internal and external customers by providing information and answering questions/requests in a timely manner.May participate on committees (both internal and external), in meetings, and work on projects as assigned.Required:Master's degree in behavioral science, rehabilitation, social work, counseling, psychology, or related field and/or license in Human Services field from accredited College or University.Registered with DORA as a Candidate working towards licensure to be providing therapy.At least one year of experience in the behavioral health field.Preferred:LCSW/LPC; CAT/CAS/LAC certification; Trauma Informed Care experience.Bilingual/bi-cultural preferred.At least one year of experience with individuals with mild to severe mental illness, IDD, and substance use disorders.Some positions include driving a company vehicle and require a valid Colorado driver’s license.Start With YesAt SummitStone, we show up not only for our clients, but for our staff as well.
