General Summary of PositionAs a part of the HEART Team, the Registered Dietitian is responsible for collecting pertinent information, development of appropriate nutritional plans, monitoring the status of patients, counseling patient and family on special nutritional requirements, consulting with other health personnel (physician, nurse, pharmacist, care manager, therapist, etc.) as appropriate. The HEART Team Service is a holistic, interdisciplinary, community based medical and behavioral health service designed to help beneficiaries manage complex medical and behavioral health illnesses. It aims to restore health, improve community, daily living, personal, and social skills while reducing incidence of medical or behavioral health crises that lead to unnecessary use of the hospital system.
Primary Duties and ResponsibilitiesConducts assessment for patients to establish diet and food guidelines.Develops and implements an individualized plan for medical nutrition therapy in accordance with patient's medical problems, goals, and objectives. Reviews patient medical orders.Calculates nutritional requirements and necessary diet restrictions or food/drug interactions.Assesses patient's knowledge of diet and provide education appropriate to patient's learning needs regarding the relationship of diet and the impact on the treatment process.Responsible for developing or obtaining patient nutrition educations materials.Documents nutritional care plans, which are a part of the holistic care plan, and patient response in patient's medical record, including nutritional assessment, medical nutrition diagnosis, recommendations, and the plan of care.Experience with Diabetes, CHF, COPD, HTN.Provides patient with ongoing nutrition assessment and outcome-oriented nutrition counseling necessary to assist patient in achieving and sustaining an effective nutritional status.Communicates and coordinates with physician and other health care personnel as needed on the nutritional care of the patients.Serves as nutritional care resource for HEART Team members.Contributes to the achievement of established goals and objectives and adheres to department policies, procedures, quality standards, and safety standards. Completes annual Code of Conduct, Compliance, and other mandatory training. Complies with governmental and accreditation regulations. Drives and travels to community locations, various agencies, and other outreach destinations.Is responsive to and communicates respectfully with patients/families, HEART Team members, and other health team members. Participates in regularly scheduled caseload consultation.Provides services remotely, in-office, telephonically, via telehealth, in the community, or beneficiary's home as beneficiary needs and preferences dictate.
Minimum QualificationsEducationBachelor's degree in dietetics, nutrition required
Experience1-2 years in healthcare required
Licenses and CertificationsAmerican Dietetic Assoc - ADA Licensed in Maryland Upon Hire required andCDE - Certified Diabetes Educator Certification Upon Hire preferred andCNSD - Certified Nutrition Support Dietitian/Clinician Upon Hire preferred
