About the unit2 East is a Surgical Critical Care Unit, specializing in cardiovascular surgical interventions. We also provide care to patients requiring general surgical interventions. Our nurses possess skills that allow them to demonstrate immense critical thinking and physical assessment skills which sets them apart as they continually manage acutely ill patients. The unit staff is comprised of RNs (registered nurses), NTs (nurse technicians) and CAs (clinical assistants). It is led by a Clinical Nurse Manager, an Associate Clinical Manager on the off-shift, and a Nurse in Professional Development Specialist. About the unitThe Medical Observation Unit consists of 54 beds. Patients are categorized as Observation secondary to insurance requirements. This is a medical unit. Patients are admitted from the Emergency Department with mental status changes, long term placement needs, pain, cardiac, TIA, r/o CVA, UTI, pneumonia, geriatric, and behavioral issues. If patients meet criteria for higher level of care they will be transferred to the assigned progressive or inpatient unit. Staff complete an assessment and shift documentation according to the Observation documentation policy. There is rapid patient turnover on this unit between patient discharges and transfers. About the unit3 East SICU is a general surgery floor and we typically take all general surgery patients, vascular, thoracic, and non-neuro trauma’s. A unique patient population that is specific to 3 East only is our Transplant, high risk OB, post-partem, and our ECMO Patients. Corewell Health-WBUH specializes in Kidney and Liver Transplant and 3 East is the first unit they come to after surgery. 3 East is also the only unit that receives ECMO patients throughout the whole Hospital System. 4CN is a twelve-bed unit that provides care for a diverse patient population including, but not limited to, respiratory, cardiac, and neuro illnesses. 4CN has the capacity to care for patients with ventilators. About this unit 6NE is a Progressive Care unit where RN’s have the opportunity to be trained to work with more critically ill patients than on a general Med Surg unit. All RN’s on 6 NE receive Critical Care training. APP’s are on the unit for both day and night shifts and participate in rounds along with the RN’s and physicians. We take care of a variety of patients with medical conditions and co-morbidities. We specialize on the acute care of the elderly. In addition to being a Magnet hospital, our staff are NICHE (Nurses Improving Care for Health System Elders) trained. Additional specialty training is provided to our staff in the care of geriatric patients. Through NICHE, all our nurses are Geriatric Resource Nurse (GRN) trained and all our nursing assistants (NAs) and clinical assistants (CAs) are Geriatric Patient Care Associate (GPCA) trained. About this unit4 Center is a 22-bed surgical unit which specializes in patients with acute and chronic kidney and liver disease. We care for patients going for kidney and liver transplants and those undergoing nephrectomy for the purpose of donating. We admit our transplant patients prior to surgery and care for them post operatively after they are downgraded from ICU. Patient education is a large part of what we do, and we work closely with our Transplant Medical Team. We are very proud to be #1 in the nation for kidney transplant survival at one year after transplant. Our nurses also perform peritoneal dialysis. We do not perform hemodialysis. About this unitWe are a 64 bed Inpatient Rehab Unit that sees a variety of patients who need to work on both their functional and medical problems. We treat ortho patients recovering from hip fractures, complex knee replacement, back surgery with neurologic involvement, multi-trauma, and other orthopedic issues. We also see neurology patients recovering from stroke, brain and spinal cord tumors, Traumatic Brain Injuries, Parkinson’s, MS, and any cancer diagnosis with functional decline. We also have patients come to our unit who had amputations and other complex medical/surgical issues with functional decline. Patients participate in 15 hours of therapy across the 7-day week. Nursing continues the interventions practiced by the patients in therapy sessions, to continue safe mobility. For many of our patients, Rehab is the key to getting back home with their families. This is a very rewarding unit to work on, as we all get to take part in the progress patients make towards their goal of getting home.*This is an office position that involves working with payors, providers and patients to obtain authorization for inpatient rehab services. Corewell Health Proton Therapy Center is equipped with a state-of-art compact single room system, IBA ProtesuONE which is the second of its kind in the world. These interventions may include, but are not exclusive to: alternate site coordination, discharge planning, addressing all forms of identified abuse/neglect, adoption, substance use disorders, assisting with end of life decisions, providing appropriate referral resources, bereavement support, adjustment counseling, facilitating guardianships and behavioral health concerns (including psychiatric transfers).ESSENTIAL DUTIES:Conducts psychosocial assessments with patient and or family to assist with identified psychosocial needs or who may be identified to require an alternate site of care post discharge and executes a plan. Communicates with alternate sites of care and/or community agencies in an effective and timely manner, to best address the patient’s needs. Completes and documents the assessment and plan in the electronic medical record, accurately reflecting the patient’s current condition, situational factors, transition of care needs and psychosocial imperatives. Provides consultation and resources to members of the healthcare team. Responsible for maintaining relevant and current knowledge of community resources. Participates or assists in department or hospital committees, projects etc., as assigned/requested. Develops and maintains current knowledge of federal and state regulations as they pertain to roleInpatient psychiatric duties: (when exists) Conducts daily group therapy sessions with eligible inpatients and Partial program patients and also may perform utilization review for this patient population Provides discharge planning and utilization approvals for post inpatient care.STANDARD QUALIFICATIONSTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily. LMSW expected within 5 years of employment.Other Qualifications:Demonstrate knowledge of growth and development and dynamics of behavior for age related groups. Must demonstrate excellent oral and written communication skills. Must be proficient in Microsoft Word and ability to adapt to technical demands of electronic communication. Ability to work independently but contribute positively to team performance. Ability to manage, prioritize multiple situations and tasks while remaining calm.
