Treatment includes individual and group settings.Completes timely documentation of daily treatment notes, progress reports at funding required intervals, initial evaluation, discharge summaries, home programs, and consultation notes as indicated.Monitor individuals’ progress and maintain accurate records of treatment through data collection and treatment reports as assigned.Serves as a member of an interdisciplinary team for assigned individuals. Participates in individual goal development, coordination of services, discharge planning, and family education as neededProvides education and monitoring to direct care staff, families, and other team members, in the provision of speech and language activities; trains and supervises staff in the provision of speech assistant services if employed at location.Attends and verbally presents at team meetings, clinical reviews, staff meetings, and in-services. Attends department meetings. Participates in program development.
